Talk to him. Tell him you've noticed that he has bad breath and ask if he's had it checked.
Constant bad breath can be a symptom of something else, so it's not a bad idea to go see a doctor.
It could be that he's just not meticulous about dental hygiene.
Either way, communication is the best course in a relationship. Tell him how you feel, be nice about it, and then try to find a solution together.
Good luck.

You said he's "aware", has he questioned why you don't like kissing him? If so, how have you responded?
The answer says a lot about the depth of the relationship because honesty is a must in a relationship where you say you're in love.
It would be best to gently advise him to see a dentist because it could be as Altenweg pointed out, a lack of a good dental hygiene routine. But there are a number of other issues that can cause bad breath; infection, stomach bacteria, inadequate saliva production, etc.. If this situation is chronic, it's best that he be seen by a dental professional for a check-up to rule out anything serious.
