H-1B is consular processing type so it's best to apply at US consulate in your home country. Visa term depends on contract offered by your employer.

If your passport is expired while you' re in US, you can renew your passport at the Pakistan consulate in US, visas in the old passport will be endorsed onto the new one, and you can simply hand both to immigration officer and you can get a new via stamp when you renew your visa OR you obtain a new passport before proceeding with H-1B.
