My ex father in law added my children on his insurance policy when he had temporary custody of my children and I had insurance on my children at the time he added them under his policy. Is this insurance fraud? Now my ex wife and I have joint custody and the grandparent still has my children on his insurance and my ex is using the insurance I have on the children as secondary and her stepfather's as primary. Is this insurance fraud? What I can do about it?

If he had legal temporary custody it is entirely possible that there was no fraud involved. I note that you are also interested in reporting someone for employment fraud. I don't know why, quite frankly, you would do anything that could cause your children to lose their health insurance.
