Possibly one quick way to make sure that you have all the pertinent information included so as to protect yourself might be to call another moving company and ask for a quote and a copy of their contract.
Or, see a lawyer, might be a good idea.

Its an extremely important document. You might be in business for years before you come to discover that you didn't cover something very critical and costs you a bundle.
Many companies (I don't know about movers) won't give you the contract until you are about to sign. Your safest bet is to find a good lawyer, but he has to know the moving business or you can do as well yourself.
This site is not specifically about contracts but it may give you some ideas of at least some of the items that need covered: Household Moving within Florida - Division of Consumer Services, DOACS (http://www.800helpfla.com/moving_text.html)

Yes, a moving service is a very very serious business, one claim can cost you 10's of thousands of dollars very easily.
And if you are ever going to move someone across a state line you have opened an entire legal issue of contract and of licensing and insurance needs.
