Do you have a spouce that wants to adopt? If not then his rights will not be terminated.

A father can not just "sign over his rights" you may be able to take his rights away for some specific reason according to your state law. For example sometimes long term prison.

Also if the court does allow him to sign over his rights, he will still have to pay child support unless you have a new partner ready to adopt.

There are no "forms" he will have to hire an attorney, petition the court, and get you and the judge to agree, it seldom happens.

I have legal court ordered guardianship of my two eldest granddaughters. My daughter and son-in-law want to sign relinquishment of parental rights. Where do we get the legal forms?

You can't. (there aren't any forms) You would have to adopt them from your daughter and son in law. Which means you get an adoption lawyer (yes you have to have a lawyer) they will do the paperwork and then a judge will sign off on it but you may have to go to court but adoptions really aren't that bad as long as there isn't any fighting.

No there is no free way to do it. And for an adoption your going to have to find out who the father is.
