Hi,

My name and social security number are incorrect on my W-2. Should I file as is and make any attachments of proof?

Any help would be great

First, try to get the W-2 corrected, even if it means filing late (no problem if you are getting a refund.

Then, if the employer will not correct the problem, file as is and attach proof of real name and SSN (a photocopy of your SS card is good proof).

HI ATE,

My marriage status on my W-2 is not updated... I joined the company while I was single; I got married in mid Oct 2005 but the company didn't update for me on W-2. Will it matter? I noticed my husband's W-2 doesn't specifically show marriage status. Thanks a lot for answering my question.

Besides, I got bank interest last year. I would like to confirm if I should file schedule B and fill out line 8a on 1040 even the amount is very little -- only $4.

I also got very little amount of dividend income. $13 and withhold is $4. It came with form 1042S since I was foreign person when I was still student before June 2005. Which schedule and which line on 1040 should I file? Since it's not from 1099-DIV, I'm a little worry if I can still file schedule B and put it in line 9a.

Please advise. Thank you!!

No, it will not matter.

Yes, report the interest and the dividend income on Schedule B. Attach the Form 1042-S to prove the withheld income. Add the $4 withholding to Line 64 of Form 1040.
