Ok I have 2 children, I am on a limited income and I am unable to afford a lawyer, I have researched and been given advice by a paralegal, (not currently working is field do to a new born ) that I have grounds to terminate. The Non-custodial parent is a dangerous, evil and a vindictive person, which has an extensive history of violence including but not limited to Assault and Battery on a minor child, and is currently incarcerated at this time to the best of my knowledge. What I am in question of at this time is how or where do I find the form (hopefully free only for EXAMPLE) " Petition and Affidavit to Terminate parental rights of the Non-custodial Parent " ? I just want to have something to go by to draw up my own petition, so I can file it myself. If I do everything in a correct manor including wording, I am absolutely POSITIVE I will succeed in this attempt. I do have physical custody and have had since 10-1998. The prison issue is NOT the only grounds that I have. It is a long list, so at this time I will not list them. I am in hopes that SOMEONE will not tell me " I don't have grounds to Terminate " I just wish for direction to some kind of a legal document library or download site with the appropriate forms so I can begin my Petition. Thank you for taking the time to help me to no longer live in fear for my children and of myself!
