The chain came out completely out of fluorescent light fixture, what do I need to replace, or how does the chain get back in there

Are you talking about the chain that the fixture hangs from (like in this photo (http://images.hardwareandtools.com/P/5953310.jpg))?
If so, then there should be a hole in the top of the fixture and you can use a S-hook (http://www.pacificplayworks.com/cart/images/H140.jpg) to connect the chain back. I've fashioned these out of an old coat hanger--just cut off about 3 inches and bend into an S shape.

I hope this makes sense.

I believe you are referring to the chain on the pull switch? If so it has one mounted from the inside. You have to replace the whole switch and it is cheap anyway. Home Depot/Lowes has them.
