Here's the file. I had to put the tables in Excel to attach them here.
Just for background, this is for a client with various compensation plans.
The SOP table (Stock Option Purchases) is for a client that offers Stock Options to employees. It would have others like it with mostly the same information, just specific to other products (ESPP, SAR, RSA, RSU, Performance)
I've also included a screen shot of the first page of my form after putting in a new client
