I am presently on F1 status and have been staying in US for less than 3 years (International student from India). However, social security taxes and medicare taxes have been withheld in my W-2. I know that I can get a refund from my employer, if not, then by filing Form 843 and 8316.

I am going to file Form 1040 NR-EZ. But the question is: Do I have to include social security wages and medicare WAGES in the form? If yes, how? What about withheld social security and medicare TAXES?

Will I be able to file my tax returns with this W-2 or do I need a new one?

In worst case, can I extend filing date of my taxes?

Any help would be deeply appreciated.

Thank you in advance.

You report wages in box 1, and your tax withheld is federal tax in box 2.

For state tax return you need info from box 15, 16 and 17, and for local taxes from box 18, 19 and 20.
