I am on a F-1 status since Aug 2006. I have been working full-time with my employer in NJ since Jun 2007 - present, on a CPT for the first 3 months, and since then on an OPT that expires Sep 2008. My payckecks have been automatically withholding social security and medicare taxes. Which tax forms do I file for federal and state taxes, and for the refunds that I qualify for? Thanks

You need to file Form 1040NR-EZ for the 2007 income tax return.
The Social Security and Medicare taxes should NOT be withheld for someone on F-1 and working either CPT or OPT. Ask your employer to refund these taxes.
When they say no (which they will because the tax year is over), get the refusal in writing, preferably on company letterhead then file Form 843 with the required support documents to get the Social Security and Medicare taxes refunded to you.
