Here's the situation. I was born with a name. My birth mother gave legal guardianship of me to her aunt and uncle when I was 2 1/2 weeks old. My parents then gave me a completely different first, middle, and last name. I was never adopted, and never went to court to have my name changed. From the research I've done, my name should be legally changed due to usage. My birth certificate has never been amended to reflect the name my parents gave me. I am needing to get a passport to go to Mexico, and am wondering if I am going to be denied due to my names not matching up. My birth name has never been used for anything. Ever since my parents took custody of me, I've gone by the name they gave me. I tried to locate my early school records, and was told they were destroyed. I do not have a Baptism Certificate. I am well over the age of 21, so any medical records from my youth have been destroyed. Any suggestions as to what else I can provide to obtain a passport. My departure date is in one month, so going to court and legally changing my name is not an option.
