You can work past that gag reflex over time by a process of desensitization. Every day for a few moments you get it to a position where you would gag... hold it as long as you can then pull away. Or in a smooth motion go past that point and out before you get a chance to gag.. You only need to do this once or twice each time... over a period you will find you can do it deeper and longer without being as quick to gag. Eventually you will have control of the reflex... but you have to do this with him just laying there. You need to be in complete control as to when, how fast and how deep for this to work. Its 90% mental.

As to sore neck? Try different positions. Everything from being towards he feet, to along side him, and depending on your weight to laying on top of him in a 69 position. One or more of those will be a more comfortable position for you, they all obviously can't be.
