If you really want to do this yourself, you can get the Form 1099-MISC and the W-3 (Transmittal of Wage and Tax Statements) from the IRS. You have to go to an IRS office to get them, since they are multiple forms with carbon paper; the forms that you download from the IRS website are for instructional purposes only.
You can also buy the complete packet from either Stables or Office Depot (and probably Office Max or any other business supply store).
Or you can hire your local tax professional to do it for you. Call a few and ask them what they would charge to prepare this form. I charge $50 for the W-3 and up to four Form 1099s, plus $5 extra for each additional Form 1099. If you are interested in having me do it, contact me at

In either case, you need to download Form W-9 and have the contractor fill it out and return it to you. The completed W-9 has all the info needed to accurately fill in the Form 1099-MISC.
