Need to legally change my last name back to my maiden name how do I do that

Just how you did it when you got married but in reverse.

You are going to need to take your divorce decree and file for a new social security card, then take that and your divorce decree and get your drivers license changed. Make copies of your signed divorce and send it in with a typed request to change your name for all your bills. The bank will need you to resign paperwork.

But everything you did to change your name when you were married, do the same but instead of using your marriage certificate, use your divorce decree.

Also, it can be done at the time of divorce. Something your attorney can take care of.

Depending on where you live, I would go back to the court house or your local tag agency and ask them if you can change it there. Generally they ask you if you want to change your name when you first get the divorce approved. I think your Court house will give you the correct facts for your area.

I am Canadian but I have been married for 39 years and at no point did I ever sign papers to change my last name to my married name... I just called myself Mrs (husband's last name). My driver's license is in my married name, as when I renewed it after my marriage I just used my married name, my social insurance number is in my maiden name (that is how I am registered with the government) and my passport is in both names, hyphenated... my point in all this is that in Canada we don't legally have to sign anything to change our last names upon marriage, as long as we are not trying to defraud it is legal to use your Birth sir-name or your Married sir-name or hyphenate them. My female aquaintances who have been divorced just reverted to their Maiden names if they had that desire. I figure if the government of Canada knows who I am and can cope with two sir names then it must be kosher... hope this helps a bit
PS if you didn't go to court after your marriage and legally have a judge change your last name then.. don't worry... atleast that is what I think!
