I spend a lot of time out of the country. What banks offer the lowest or even zero foreign ATM use fees?

They are usually around the same. It depends more on the relation of your bank with the other bank or ATM , because some banks have deals or belong to the same "circuits" and in that case the fees are less. The best way to avoid fees altogether is to simply use your ATM directly on POS machines:)
