    Did not receive 1099-R yet?
    I made a withdrawal on my 401k this year and did not receive a 1099-R. I have the payment record and it says "If applicable, an IRS form 1099-R will be sent to you early next year". Do I include this information this year or wait until next year? I had taxes taken out already.

    If the withdrawal was made in 2009, you must report it as income on your 2009 tax return. The institution is supposed to mail out all 1099-R forms by Feb 1, 2010 - so maybe it's in the mail. If you are anxious about this, perhaps you can download a copy from your acount on the firm's web site, or you can call to verify they sent it out.
