Surnames and Hyphenation formalities

I am wondering what the correct formal address would be in correspondence for a woman who hyphenates her surname with her husband's name eg

Mr J Hibbert and Ms Y Jones-Hibbert or

Mr J Hibbert and Mrs Y Jones-Hibbert

and singularly:

Mrs J Jones- Hibbert ?

How would this be made clear to friends and relatives following the wedding that the Bride was adding her husbands surname rather than replacing her former name with his?

How would a couple be formally introduced?
