changing visa type from F1 to H1B

Hello,

I have a question regarding changing my visa type.

I currently have an F1 visa with an OPT that allows me to work in the US. I have a job in an American company and they applied in my behalf for an H1B visa.
My application was accepted and approved, so my questions are the following:

1. Do I have to leave the country in order that my new H1B visa will come into affect, and if so, do I have to go back to my home country or can I just cross the border and come back from Mexico or Canada?

2. I currently have the form i-765 that states that my H1B petition was approved, how do I get the new visa on my passport, can I do it in while I'm in the US.
