W7 - ITIN - Passport

Hi,

I am on H1b and my wife is on H4, my wife came to USA immediately after marriage, her passport was in her maiden name and visa is also in her maiden name, after coming to usa we got a new passort for her from indian embassy in USA with the changed last name, now
1 ) which passport copy should i send to for ITIN alongwith W7 and
2) what pages of the passport should i send

The name i would like on the ITIN would be the changed one since in future i would be applying the visa extension with the changed name,

and also the New Passport is the current official Passport of hers, and the visa is stamped with the old passport as per your advise i should be sending copies of the previous passport since the visa is stamped on the previous passport.
