tax filing resident in both US and India

I need to file income tax return in India for A.Y. 2012-13 however have a specific question.

For Indian FY 2011-12 I was in US from 1st April 2007 to 10th Sept 2011. Thereafter I worked in India from 11th Sept 2011 to 31st March 2012.

I recieved W2 in US for US FY 2011 and filed my taxes in US.

Now I need to file income tax return in India for AY 2012-13.
Do I need to declare income I had in US and tax I paid in US??

If yes, then how? US salary and tax is in dollars and in India amounts needs to be in rupees. What conversion rate to use?
Also I won't have TAN number for US employer.
How do I put salary and tax info on "TDS" tab when filing my tax online?

Kindly guide me on how to proceed.
