A green card holder must file resident tax return.
For foreign income, you do not need W2 or attach it. Only if IRS enquires you, you will show proof like bank statement or salary statement.

Like MukatA said, as a green card holder, you must file a resident tax return (Form 1040).

You should be able to file the Foreign Income Exclusion (Form 2555) to exclude most, if not all, of your Canadian income.

If you make more than $85,000, you should also be able to claim the Foreign Tax Credit (Form 1116) to get credit for the Canadian taxes paid on the amounts OVER $85,000.

I suspect that you will owe NO U.S. taxes at all.

To file either Form 2555 or 116, you MUST file Form 1040.
