You use NO form for 401K.

It is documented on the W-2 and is NOT reported on your annual rax return.

Previous posters are absoultely correct, unless the question has to do with figuring taxes due on a distribution from a 401(k). Distributions are reported on form 1099-R, and in this case 1040-EZ can not be used.

Ebaines answer is accurate. If the 401K issue is one of a distribution, then you must report the distribution on Form 1040.
