warranty deed

I have a property in the state of New York and would like to sell it. The title is in the name of my uncle who had sold me the condo a few years ago but title was never transfered or recorded in my name. My uncle as well as I live in the state of Florida and I would like to draw up a warranty deed for my uncle to sign over to me. What would I need to do so when I sell property the proper proceeds will go to me and not my uncle? Does the caption on warranty deed need to be
ex.:

STATE OF NEW YORK

DUPAGE COUNTY

OR

STATE OF FLORIDA

SEMINOLE COUNTY
