Tap Water hot then runs cold after 6-7 mins

I have a Worcester 924 combi boiler. the central heating works fine. The problem is with the hot water. i need to switch the hot tap onto nearly full to get the bolier to kick in..and the after about 7 mins (while filling the bath up) the hot water will run cold for about 2 mins then start heating up again, but never gets very hot after this. Any ideas ?
