MN Trampoline Liability Waiver Form

I don't find an explanation or example of language that is "required" but you're right to be thinking of using a Waiver. See this MN Court Case:

Here are some terms for the waiver (scroll down to ATHLETE WAIVER/RELEASE FORM ("AGREEMENT")) that you can use that should be acceptable...but if I were you I'd get any minor's parent AND the kid to sign it before allowing them. I'd also post a no trespassing/danger sign outside of it.

Of course an attorney will give you the best answer, but from the looks of that court case I personally would be comfortable enough doing it on my own.

You may also want to consider some strict rules for the use of your trampoline to minimize the risk of injury. I have put together some of the more important rules people to post on the fridge.

Trampoline Safety Rules

Practicing good safety habits is essential to having a fun and safe time with your trampoline. Using the word "TRAMPOLINE" can help you remember some of the most important safety rules.

Try only simple skills. Advanced level tricks should only be attempted with a trained coach and after the proper techniques and progressions have been learned.

Remain in the center of the trampoline at all times. Jumping near the edges can be dangerous.

Adequate spotting should always be present (one person per side). Never jump without someone else nearby.

Make sure that the area beneath the trampoline is free from all objects and/or people.

Place your trampoline on level ground, away from low tree branches, power lines, your home, etc.

Only one person should jump at a time!

Leave other objects off of the trampoline.

Improper stopping techniques can lead to injury. Always use the proper stop.

Never jump off of the trampoline.

Enforce these rules. Keep your children safe and be happy to know that they are practicing good trampoline safety habits.

The trampoline can be a lot of fun but it can also be very dangerous if these safety rules are not followed. Please understand that these rules will not prevent accidents but they will help to minimize the risk of injury. Remember, we all want our children to grow and learn in a safe and positive atmosphere.
