IRS Reporting of 401K distribution as part of divorce settlement

A friend was divorced in 2007. As part of the divorce decree, a portion of his 401K was transferred to his ex-wife via her attorney. My questions:

1) Does this disbursement need to be captured when filing taxes?

2) If so, how is this reported on the tax form?

3) What documentation if any is needed when filing taxes.
