Hello,I need to find resources to file a quadro,I have a model related to the type of retirement but I need the blank forms,this applies within the state of California,otherwise a reference about who can do it for me at a reasonable fee.Thanks .

I found Thomas at this website to be very helpful and knowledgeable. I am still having difficulty getting my QDRO straightened out. According to the online Court file, the QDRO was filed with the court Clerk. However, there are no documents in the actual file. It seems that my ex's lawyer never completed the job.
