Son wants to change last name

MY 17 year old son wants to change his last name to my maiden name. How does he go about it? He will take the my maiden name to go on. He is the last of relative of my father who is gone now and I'm the only one that had the last name until I married. My children now have my husbands last name. My son wants to change it back. So what does he go about changing it back? P;ease let me know so we can get this done for him.

Where are you from. Here in canada you go the the Canadian government website. If you want to change a name. You need to fill out forms and pay a fee. If he is under a certain age it has to be agreed upon and signed by the guardianship.

In the US a person can use any last name they want to as long as its not used to commit fraud. But if he wants to legally change his name, he will have to submit a petition in family court.

changing your name is very simple, you file a form with the court and the judge signs it,

Since he is a minor, (under 18) both parents may have to sign for it, or he may have to wait till he is 18
