An example of one that I have had to complete in the past:

First: Introduction
-Identify importance of the study and who is authorzing the report.
-Why needed
-Discuss the issue
-Implications
-Present tense
-Paragragh format (1-2 paragraphs)

Second: Purpose and Scope
-Identify general goal/purpose of the report/letter, as well as problems (sub-problems)
-1-3 paragraphs

Third: Procedures
-Procedures used to gather data
-Past tense or passive voice
