Shoulder Pain

I have a terrible pain in my left shoulder.Towards the front.It hurts when i breathe or cough.I can lift my arm with out pain though. It just started today after a traumatic thing happened. had a blood clot in my left arm 11 yrs ago.,and I am on coumadin. It is not red or swollen and i can feel it at the top of my back also. I have had bursitis in my right shoulder years ago.I am 55 yrs. old and in reasonably good health. Can this be at all stress related. What should I do?
