This could be a number of things, especially with the drinking and smoking. But HIV would not be one.

He should see an internist who may or may not check him for Anorexia or something less common, but commonly misdiagnosed called Achalasia.

How old is your frined? Having diarrhea for more than one week needs to be treated by a doctor. Make sure to have them drink water and gatorade to keep the fluid and electrolytes balances.

I don't think this is related to HIV however the signs/symptoms are:

Sore throat, fever, swollen glands, and rash. These symptoms usually clear up in a few weeks. However you can remained without symptoms for 10 years.

smokes and drinks-sounds more like alcoholism-the food is the drink....I think his issue may primarily be psychological.

P.S. constant diahorrea IS a symptom of advanced HIV (aids). If he hasn't told you by now he has AIDS, then he probably doesn't have it. if you are at the stage of constant diahorrea-you are days away from death. Highly unlikely he's that ill.

Not much to go on
Gender ?
age ?
medications ?
how long has this been going on ?
Does it occur with liquids or solids?
Trouble or painful swallowing ?
Only with alcohol ?

but pancreatitis, esophagitis, peptic ulcer, gallbladder conditions, diabetes, drug withdrawal are just some of many possible causes.
If HIV is a concern get tested.

Alot of people that smoke and drink, do not deal well with stress. My first thought was colitis, IBS. He really needs to see a doctor.
