opt to H1-B

I finished school 2006 Spring and did OPT until June 2007. I work under H1-B from last October. Should I use 1040NR or 1040R form?
Since I've been working at the same office, I have one W-2 form through the year. Does anyone have idea how I can claim for Social Security and Medicare. I could get the amount for the wage for them till June; however, no official documents except payment status.

I really appreciate your help.
