Hospitals that are on Medicare approved lists have agreed to accept the amount of money medicare will pay ( agreed contract pricing)
So a person with no insurance may have to pay 10,000 for a treatment, but medicare has a agreed to price of lets say 5000 for that, which the hospital has to accept. So medicare will pay only thier amount

So non approved hospitals will get the 5000 dollars, but still bill the full amount of 10,000.

** in general if I understand what you are asking
