what do the numbers 21 and 91 mean on the Patriots helmets?

Those added numbers are often in rememberance of key players, either on that team or elsewhere within the league. It's a tribute.

Number 91 is Marquise Hill the Patriots player who passed away this past summer!!!
The '21' is in honor of former Redskins safety Sean Taylor who was murdered
