What can I do when my ex is withholding my visitation with our child?
A judge has ordered that I have supervised visits with our child due to a DV incident that happened between the mother and I seven years ago. I have never missed a visit with my child and have made requests to see my child at least once a week. I am the petitioner in the custody trial less than a month away. Recently the mother has not responded to any requests for visitation and I feel she is withholding my daughter. I haven't been able to see or talk to her for two weeks now and I fear she may be making plans to run off with her. What can I do?

There is really little you can do for now. Document all your attempts at visitation that fail, to take to court. If you can afford to hire a PI, find out what she is doing that may indicate a plan to disappear. If you have a lawyer, let him or her know about this.

What evidence do you have that she plans to run off?

Having a feeling is one thing; evidence is another.

I would ask the court for an emergency hearing to have her held in contempt for not allowing visitation. At the hearing ask for a restraining order preventing her from moving until the custody issue is resolved.

What is on the table as far as the custody hearing? You might want to file for contempt as of right now if she isn't following the orders of the court and may refile (amend) your petition to reflect the recent events depending on what your asking for in the first place.
