I understand exactly what you are talking about.
The problem is that each state has it's own forms and may not accept a general form as evidence of transer of title. Do a search on Department of Motor Vehicles. Include your state. For example: New York State Department of Motor Vehicles. It should take you to their website.
Search in the transfer of title area to see what forms are available for sales tax exemptions. If nothing fits, then I would suggest next week, call the phone number listed on the web site. Explain the situation. If this is part of your divorce agreement, they may want to see a copy of your agreement and a letter signed by your ex stating his intentions to sign the title over to you with his signature notarized. But, let them guide you. The biggest problem is that a lot of these people that work for the DMV, don't even know what will be acceptable! Someone may tell you one thing and you find out later on, that it was not the way to handle it. So, when you call, ask to speak to a supervisor, or someone who handles transfer of title. Hopefully you will get someone who won't steer you wrong.
