    Birthname is not the same as social security card
    Hi, my problem is when my son was born in 1991 they sent off for his SS card. Instead of putting the SS card in his birth name Allen, they put it in his father last name which is Jackson, only because there was a paternityship paper showing that it was his child. I did not know they did that until it came in the mail in Jackson. Now I went to get ID for him and I never had the Birth Certificate changed in Jackson and in school he use Allen all the way through school. The DMV say in order for him to get a ID he needs to change his Birth Certificate to Jackson or SS card to Allen. Well he is 18 now and he would have to go through the courts to get his name put back in Allen. How do He go about doing that. They say I can't do it but my child has been in Special Ed since he was little and he can't do that by himself he don't understand. I need help please somebody tell me what I need to do to get his name back in Allen or Have the name on his Birth certificate put in Jackson. All his medical cards are in Jackson based on his SS card. This is a mess.

    Actually, it's legal in a lot of states to start using a new name - there's no need to go through the courts.

    Your son can file a new form to have the name on his Social Security card changed. You can fill out the forms, but have your son sign where required as he is a legal adult. If he has been diagnosed as a doctor as being mentally handicapped, then you can fill out the form as his legal parent and guardian.

    And yes, this is a mess. This should have been dealt with 18 years ago, not now when your son is in need of an ID and is unable to obtain one. If you noticed the error when you received the card when he was still an infant, you should have completed the forms back then.

    Thank you I said the same thing. He was diagnose with a learning dissability. He has an appointment with the SS tomorrow and I can have him fill the forms out then while we are doing his redetermination. Thank you so much!
