Activating a Recovable Trust

Two questions:
1. Once a trust is funded, is it necessary to "register" or "record" the trust with a county clerk of court or some other agency?

2. If I place mutual funds and other investments into a revocable trust,...
a. Does the Trust have to have a separate EIN?
b. Do any stocks or mutual fund accounts have to be renamed as the Trust?
c. Does the Trust file an annual tax return; if so, what kind of return?
