Disappearing Icons (Outlook 2000)

Every time that I customize the toolbar by adding a new icon in Outlook 2000, it (the icon) is gone the next time that I launch the software. For instance, if I add the Spelling icon (found in the Tools menu), it is no longer there the next time that I start up Outlook 2000. All the other icons are still there. Only those that I manually add do disappear...
What could the cause be?
