Form to Defend Action by Creditor

I need to find the form called "Notice of Intent to Defend". I am not sure where to obtain this form. I am trying to help my daughter out of a tough spot where she has had a summons sent to here from another city regarding an old credit card debt. Thanks for any help.

no specific form is usually needed. A simple letter stating that you intent to defend against the suit will be sufficient. If any form is required, just contact the court clerk for how to obtain one.
