Door distance from stairs

How much distance is required between the highest stair inside and an indoor/outdoor egress door at the head of the stairs? This door would be an emergency exit for the 2nd story level as well as access to an elevated porch.

Where? What State? I'm an accident investigator - it varies State to State and, for that matter, locality to locality.

I spend a great deal of time measuring stairs and determining their angle - immediately AFTER people fall.
