I'd like to change my son's last name after 11 yrs.

Hello,
I have a 11 yr. old son which I'd like to change his last name to mine. I made the mistake of giving him his dad's last name at birth. His dad lives in another state. We never got married. And his dad has spent most his time in prison. Can anyone help me on doing this?
