adding mothers last name to childs birth certificate

How do i go about adding my last name to my childs farthers last name for her to have both last names?

As far as I know you can call your child whatever you like.

How do i go about adding my last name to my childs whom has her father's last name on his/her birth certificate? can the child have both last names. can the mother's last name just be added on to the birth certificate?
