This kind of problem is really a case for medical specialists, and not for well willing and well intended strangers on a Q&A board.
What does your physician and the specialists involved say about the progress and prospects?
And to be clear : what do you expect to hear from people who reply (other than what I posted here)?
I would asses his diet as this could easily be the source of the "hyperactivity".
Many children are sensitive to artificial colours, sweeteners and preservatives.
Alot of ADHD diagnoses are due wholly to poor diet, too much over processed food and little or no routine.
There are two good books on 'food allergies' causing ADHD]
The Crazymakers by Carol Simontachhi
and Brain Allergies by Dr Philpott
I would think the drs should have been able to figure out why he is not talking.
Did they ever say anything about him possibly having some form of autism?
the drs never figure out why he is not talking. some drs say that he is autistic and some say ADHD and i ve taken him to a Neurospecialist and psychiatrist. the medicine is always same RESPERIDONE,RITALIN and TEGRAL. since he started walking he throws toys from the window and till today his habit has not changed. the doctors also suggested increase the dose, when we increase the he shouts more, he eats too much.
2. Ops son may not be the only person that suffers from this...we could have members or guest that know someone or even have a person in their family with similar or the same symptoms. They could give her questions and ideas to share with the doctor.
3. I think that Nohelps and Curlybens post are excellent examples of why someone would come on here asking a question such as this.
4. I know from experience that extreme even rare medical cases can be helped from different ideas and input from many people all over the world...my son is living proof.
Sometimes doctors can be so caught up in the most serious health issue that many effects of the disease or side effects of medicines can be pushed aside and not looked at or treated. My son is living proof of that as well.
A specialist can be so focused on curing the major health problem that they forget to look at the body as a whole....my son is living proof of that as well. I might add that my son had excellent doctors treating his kidney disease and they were not infallible. Even the most brilliant and educated in their field can make a mistake or over look a symptom.

You said "he is very naughty makes too much noise."
Please understand that he is not trying to be naughty, sometimes medicines will cause tempers to flare and also cause patients to to have angry uncontrollable outburst. I think it is especially hard on young children, probably worse on your son because to start with he has problems communicating.
Also have you/doctors tried any type of communication intervention such as Sign language to help him better communicate with you. If not speak to the doctors about it, he may be less "naughty" if he can get you to understand him.
Good luck with your son and keep searching for answers.
You say he speaks in languages you don't understand. I have to wonder if schizophrenia has been ruled out. While rare in a child this young, it is still very possible.
Some of the symptoms you are describing are indicative of schizophrenia.

Unfortunately drs any more are more into pushing pills and covering symptoms and want to give a blanket statement of ADHD. I went through it with the drs with my ADHD son for years and even the psychologists and specialists didn't get to the root of the problem. In fact one even said schizophrenia and many other psychological problems can not be diagnosed in kids.
I asked another psychologist about some diagnosis' and they said, ''Well we don't like putting labels on kids it gives them a stigma". How is that suppose to fix problems when they want to ignore it?
I would continue with the drs that say it could be autism because it sounds like they are more interested in digging deeper than the mask it with some Ritalin approach.

It has some points on behavioral therapy for children with ADHD and it seems like you could use some help in that area too.

The reason this is done is solely for insurance purposes. Once "labeled" the child, as it gets older, may not be eligible for insurance due to pre-existing conditions. So, they treat as necessary without putting that permanent label in the child's permanent medical record.
