Please Help Me

Here are the symptoms- Struggles to increase in speed, RPM gauge goes crazy constantly. The engine shuts down average 10 times on a 20 mile drive. Cranks up after a dozen or so tries.The engine makes a puttering sound as if it wants to shut down, Jerks back and forth. Well maintained,252K,stick shift.Basic model. PLEASE HELP.
