Jerking Accord

My car is a stick shift. Basic model. Very well maintained.252K+ miles.The RPM gauge goes crazy constantly.The engine jerks and stalls numerous times while driving. Puttering sound as if its going to shut off. It struggles to increase in speed while the rpms stuggles to move as well. Any help is appreciated.
