If I were to format one, I would use a program like Microsoft Excel so that everything looks very neat and professional. I would then divide the sheet into sections dealing with the various rooms and surfaces in a home or office or other type of area that would need to be cleaned. I would decide if I am going to charge by the hour or square foot for the area to be cleaned.
To be competitive with others, I would just call other similar cleaning type people and ask how they figure their rates and what they would charge to clean a certain type of area if they were given a certain amount of time to clean it deeply or routinely.
