Hello rod:

I dunno. Did somebody sue you? Did you not go to court? Who sent it to you? Some collection agency, the court, a lawyer, your HR department? If you owe the money and didn't do anything about it, this certainly could be for real. But, you'll need to tell us more for any real answers.

Hello again:

I think they're gonna garnish your wages.

As excon indicated its hard to tell without seeing the document you have recieved. But it sounds like someone obtained a judgement against you and filed a court order with your job to garnish your wages. Not sure what or why you need to sign anything since the job should have no choice but to garnish your wages.
