Me and my daughters father was never really together and one day I found out I was pregnant....I'm grown so there was never a thought of not having her...when she was five months old I met my husband who she knows as her father. Know she is about to start school and I want her last name the same as ours. How do I add the name?

Your choice is to do a legal name change, that does not give your husband any legal rights to child, just changes the name. If something were to happen to you, the bio father would be where the child would be sent.

you start an adoption of the child by your husband.

Both will take the agreement of the bio father.

The adoption is often agreed to faster, since it stops his child support once it happens.
